Metro Spine & Sports Rehabilitation is an outpatient physical therapy group with locations in the Chicago Loop and Lakeview areas. At Metro Spine & Sports Rehabilitation, our number one priority is the patient. We strive to provide individualized treatment with hands-on, compassionate care. We do not rush our patients or our clinicians. Rather, we perform comprehensive evaluations and encourage patient input for treatment planning and goal setting.

Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.
Yes. A physician referral is required by your health insurance plan for you to be seen by a licensed therapist. We also offer treatments, services and/or products that do NOT require a referral and are available on a self pay basis.
We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.
As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.
Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.
– Referral slip from your medical doctor

– Your ID
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.
Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.
We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.
Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.
The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.

Not only did she get me walking again without pain (which I had been told by other offices was impossible- guess not) , but I’m back into Cross-Fit workouts, Triathlons, and Team Sports. Amazing.

“Can’t say enough about this place. They rock! I had some issues with my shoulder and Jocelyn (PT) was a lot of help. Very patient, knowledgeable and made what could have been a grueling experience enjoyable. I actually looked forward to my visits. I would highly recommend Metro!”
"I did a twisting motion while picking up a giant server in an enclosed space, so next thing I knew, my L5/S1 disc is protruding and touching one of my nerve roots. Furthermore, I had scheduled knee surgery for my ACL soon after that incident, so I went in with a double whammy. Matt takes his time to understand the pain you're experiencing, and his treatment is backed by modern science. My knee was up and better than ever before after a couple of months of therapy.
Unfortunately, my back is extremely stubborn, and after I mentioned to him that I felt my treatment had plateaued, he suggested seeking a second opinion in the form of an orthopedic specialist. I was impressed that he did not wish to further waste my time and squeeze me and my insurance for more money.
Overall, the staff are very pleasant - from the receptionist, to the PT, even the temporary ones that come in when Matt is out. They might not be able to fix every problem you have, but they'll try their best, which is all you can ask for."

"These guys are the best I've ever been to. I've seen lots of physical therapists over the last 20 years and Keith and Jocelyn are the best I've ever seen. These guys are different than your typical harpster wheel physical therapists that just give you exercises to do and then just count and stare.
They are both really good at diagnosing the issue and creating a treatment plan. Keith did a great job of adapting the same exercises everyone gave me for my condition so that they actually worked. Jocelyn has really good attention to detail that also helped speed my recovery.
I've never written a yelp review before but these guys really deserve 5 stars. They're in it for all the right reasons. It can get a little busy at times but that's only because they're so good, and they can get more done in 30 mins than many PTs can in weeks. Give them a try, you won't be disappointed."
"My mind believes that I am strong and athletic, my body vehemently disagrees; The result is frequent trips for Physical therapy. I have been to quite a few places in Chicago and can hence say with a great deal of (knowledge/experience) that Keith at Metro is hands down the best in the business. If you ever find the need to search for PT, 1) Don't ignore what your body is trying to tell you, if "you don't have time" make time! 2) Look no further than Keith at metro, as he is both knowledgeable and very good at what he does."
